Submit your Application

After you have completed all forms and gathered all documents, put everything into one package and send it to the CMLTM. You may deliver your application package in person to the CMLTM office, or you may send it by mail or courier.

Application Checklist

You must submit the following in your application package:

  1. Completed CMLTM Application form (signed declaration and information release)
  2. CSMLS or CAMLPR MLT exam results (Active (Full) registration)
  3. Copy of MLT education transcripts
  4. Official copy of CSMLS or CAMLPR Prior Learning Assessment (PLA)
  5. Evidence of Professional Liability Insurance (PLI)
  6. Canadian government issued photo ID (driver's license, passport, permanent resident card)
  7. Criminal Record Check (with Vulnerable Sector Check)
  8. Child Abuse Registry Check
  9. Adult Abuse Registry Check
  10. Evidence of English Language Proficiency (BOA-17)
  11. Applicable fees
  12. Completion of CMLTM Jurisprudence eLearning Modules (access instructions to be provided once application has been submitted)

IMPORTANT
Do not send original identification documents such as passports in the mail. Please submit photocopies of your identification documents.
